Patients have the right to:


§         Receive respectful care

§         Be given, upon request, the name of the attending physician and all other physicians participating in the patient's care as well as the names and functions of other healthcare persons having direct contact with the patient

§         Have complete information

§         Receive appropriate pain management

§         Informed consent

§         Refuse treatment

§         Privacy

§         Confidentiality

§         Be accepted for treatment

§         Receive information about affiliation

§         Refuse experimental treatment if asked to participate in an approved research project

§         Assistance in obtaining consultation with another physician at the patient's request

§         Medical and surgical services without discrimination based on race, color, sex, sexual preferences, national origin or source of payment

§         Access, when possible, to an interpreter if patient does not speak English

§         Not to be awakened by hospital staff unless medically necessary

§         Be free from needless duplication of medical and nursing procedures

§         Medical and nursing treatment that avoids unnecessary physical and mental discomfort

§         Know about hospital regulations

§         Receive information about continuing care (home care or nursing home, etc.)

§         Billing information

§         Receive information about protective services

§         Be free from restraints unless medically necessary

§         Have a family member or representative and his/her own physician notified promptly of admission to the hospital

§         Be informed of the hospital grievance process

§         Make advance directives

§         Be free from abuse or harassment

§         Easy access to their medical record within a reasonable time

§         Receive care in a safe environment

§         Be free from seclusion and restraints

§         Actively participate in their plan of care

§         Receive detailed explanation of his/her bill

§         Full information and counseling on the availability of known financial resources for his/her health care

Patients have responsibilities to:

§         Ask questions

§         Respect others

§         Keep appointments

§         Pay bills promptly

§         Give full information

§         Follow caregivers' instructions

§         Report any changes in physical condition

§         Actively participate in plan of care and education offered
